### Fixes
Remove internal tx and token balance fetching from realtime fetcher (#1724) Internal transactions and token balances are the slowest to fetch and import. We'd rather have almost instant import of almost all block-related info than frequent timeouts in realtime fetcher and handing blocks over to catchup fetcher which imports this data asynchronously anyway.

Flatten indexer module hierarchy and supervisor tree (#1718) * Add a defsupervisor macro with common supervisor functionality This macro allows removing more than a dozen of copy-pasted `supervisor.ex` modules. The `Indexer.Fetcher` module will be expanded with even more common fetcher functionality in upcoming PRs. * Flatten indexer module hierarchy and remove generated supervisors Modules are organized into 3 directories: `fetcher`, `transform` and `temporary`. Supervisors are replaced with `use Indexer.Fetcher` clause. `token_balances.ex` and block fetchers are left as-is for now, as they require more invasive code refactoring, which will be done in upcoming PRs. * Flatten supervisor tree There is no need in separate `Block.Supervisor` hierarchy, so its children are placed in the root supervisor tree. The tree itself is moved to `Indexer.Supervisor` module to reduce amount of directories. * Add CHANGELOG.md entry

Make websocket optional for realtime fetcher (#1778) Realtime fetcher already uses both websocket and JSONRPC polling for more reliable new block fetching. Though, if no websocket was available, the realtime fetcher wouldn't start, and no new blocks arrived at all. As we already have a polling fallback, we now make websocket optional. This will also help with supporting nodes without websocket API. To disable websocket API, pass `nil` in `subscribe_named_arguments.transport`.

Discard child block with parent_hash not matching hash of imported block Consider a root block `A`, old chain `-- B -- C` and new one `-- B' -- C'`. Due to asynchronous nature of realtime fetcher it is possible for block `B` to be imported _later_ than the entire chain `-- B' -- C'`. In this case we should discard block `C'` and let it be refetched. When that happens, block `B` gets discarded to make sure block `B'` gets eventually fetched and the chain becomes consistent.

Optimize token holder count updates when importing address current balances Problem: realtime fetcher transactions takes too long and often fails with a timeout. The reason of timeout is deleted_address_current_token_balances step of import, which selects previous balances for the rows we are going to update. The SELECT query it makes is extremely slow. Solution: we incorporate fetching old balance value into the upsert itself. This is done by help of a transient old_value field, which is updated in ON CONFLICT clause of INSERT with the previous value and returned. Upgrading: migration 20190321185644_add_old_value_for_current_token_balances.exs

Force consensus loss for parent block if its hash mismatches parent_hash Fixes #1644 When reorg occurs and the older of new blocks fails to be imported, the old consensus block remains in the database. Catchup fetcher ignores it, and we get a discontinuity in the main chain. This commit adds an additional consensus loss step during block import: the parent block with hash not matching parent_hash of current block is marked non-consensus, thus creating a hole in main chain, forcing catchup fetcher to retry fetching it.
