Implemented a JSON RPC method "net_services" that takes no input
parameter and returns an object
containing a list of enabled services (jsonrpc, p2p, ws, metrics) with
"host" and "port", as show below;

Implement admin_nodeInfo RPC call. This involved bringing data from the
ethNetworkConfig and genesis json into the rpc call, so it had a lot of
testing side effects.

Return world states as an optional to allow code to handle world states not be available cleanly.
A world state is considered available if it's root node is available.
Delay storing the root hash when fast syncing a world state until the download completes so it isn't considered available and skip downloading world states that are already available.

* Stream chains now take up less vertical lines, only breaking on
stream operations.
* Long annotations that span multiple lines no longer have a dangling
parentesis and indent 4 spaces.

Metrics being captured initially:
Total number of peers ever connected to
Total number of peers disconnected, by disconnect reason and whether the disconnect was initiated locally or remotely.
Current number of peers
Timing for processing JSON-RPC requests, broken down by method name.
Generic JVM and process metrics (memory used, heap size, thread count, time spent in GC, file descriptors opened, CPU time etc).

* Make GenesisConfigFile responsible for handling all the content in the genesis config file and rename GenesisConfig to GenesisState as it is now just responsible for creating the initial state at genesis.
* In CliqueProtocolController, pass the network ID to EthProtocolManager instead of the chain ID and use downloader parallelism setting instead of network ID for the number of threads.

The MiningCoordinator has been split into common, and ethhash
specific funcationality.
This adversely affects the JSON RPC, in that all mining related
RPCs are now generic based on the type of miner being used.
