This makes tracing the necessary scope of functions harder and is one of the cause for unnecessary queries/code execution. Problem: a transaction created_contract_code_indexed_at gets ignored during a conflicting update, so it is not updated. In particular it is not reset to NULL and this has been source of errors. pasqu4le	c38aea875c	Avoid fetching internal transactions of blocks that still need refetching ... Problem: blocks that need refetching may have the wrong transactions, so if internal transactions for this block are fetched they can (in some cases) fail to be inserted because their transaction does not exist. As a consequence this causes their insertion to fail multiple time before the block gets refetched. Solution: exclude blocks that need refetching from internal transactions indexing.	5 years ago

In particular this time Address controllers were checked and this (mostly refactoring) changes were made: - a few calls to `Explorer.Chain.hash_to_address` were modified to avoid useless preloads where possible - `Explorer.Chain.find_or_insert_address_from_hash` was modified for the very same reason, because it calls the previous function - `Explorer.Chain.address_to_logs` has been modified to have a more legible where clause - both `Explorer.Chain.hash_to_address` and `Explorer.Chain.token_from_address_hash` were modified to support `necessity_by_association_option`s for consistency	5 years ago
