The public permission of `:view_news` causes the Module menu to always render when the user is anonymous, even when the Setting `login_required` is enabled. We don't know for sure how duplicates can occur here, but a unique index will: - prevent data corruption - cause an error when invalid data is about to be added to the database - speed up typical access to the journals table	9 years ago

The heroku ruby buildpack used on Packager.io sets BUNDLE_GEMFILE to ./Gemfile, which was causing plugins to not be picked up because the plugin_info script was incorrectly trying to load a Gemfile from the frontend/ directory.	9 years ago
